Enrollment Fee: $75/person

Newborn - 17 yrs of age: $50/mos

18-125 yrs of age: $100mos

Family Maximum of $350/mos

*Clinic policy is to charge the $75 enrollment fee upon enrollment. This is NON-REFUNDABLE. As membership space is limited, BILLING BEGINS UPON ENROLLMENT (which may occur before your first visit). A method of payment is required and must be eligible for auto-draft. Pioneer Health DPC is a subscription based model and membership fees are billed monthly (or annually if preferred). We require 30 days notice for termination of service.
